Case Management and Support Services
The Client Services team provides a comprehensive and professional service to homeless and vulnerable adults accessing Hutt Street Centre as well as boarding houses in the Eastern region of Adelaide.
The Eastern Adelaide Homelessness Service (EAHS)
The EAHS team work as an intake and assessment service both on site and as an outreach service. The team provide information and referral to homeless and vulnerable adults seeking support in the inner city of Adelaide.
EAHS provide professional case management services that provide holistic support to the client group around a range of issues such as:
· Mental health
· Drug and alcohol misuse
· Social isolation
· Relationship and family breakdowns
· Unemployment and poverty
· Chronic illness
These services are provided with the aim that it will break the cycle of homelessness.
The EAHS team also provide referrals and partnerships to assist people who are homeless or at risk of homelessness in order to achieve the maximum possible degree of independence and community participation.
Boarding House Outreach support Team:
Hutt St Centre’s boarding house outreach support team, provide support to individuals living in boarding houses within the eastern area of Adelaide, to set goals and move towards independence, community engagement and independent living.
Other services EAHS provide to our clients:
An intensive Tenancy Support Program run in partnership with Red Cross that provides support to clients who may be at risk of losing their tenancy.
Home and Community Care (HACC):
The HACC team work with The Aged City Living and Home Support programs to provide comprehensive case management and community support services to clients aged 50 years and over (under 50 if from an ATSI background) who are experiencing homelessness within Eastern Adelaide and parts of the Western Adelaide regions.
Case Managers ensure that clients learn the skills to be independent and access the support they may need in the community and at home in order to live healthy, active and fulfilling lives.
Diversional Therapy Service:
Our Diversional Therapy Service provides coordination, leisure and recreational support to vulnerable and socially isolated adults 65 years and over (50 years and over if from an ATSI background) living in the south eastern and north western regions of the Adelaide metropolitan area.
